Coordinated Entry

Cowlitz County is required to have a Coordinated Entry system that provides a streamlined process to access, assess, and refer to specific housing programs in the community (see list below).  These specific housing programs have limited availability and program openings are contingent upon capacity and/or availability of funds.  Coordinated Entry does not provide emergency services or housing, and referrals to these specific housing programs are not immediate and happen rarely.  

If an individual experiencing homelessness is interested in one of the specific housing programs below, the first step to determine eligibility is to connect with Coordinated Entry.  A brief screening will be completed and the individual will be entered into the Homeless Management Information System (HMIS).  When a participating program has an opening, an individual who is active in the HMIS will be contacted for a potential referral.  Due to the limited capacity of these programs, only about 10% of individuals who connect with coordinated entry are able to be referred.    

The list below identifies the specific housing programs that are required to accept referrals only through Coordinated Entry. 

  • Lower Columbia CAP’s Rapid Rehousing Program (HEN/ABD eligible)
  • Lower Columbia CAP’s Young Adult Housing Program

Coordinated Entry may also provide information on a variety of other housing programs or resources in our community and can help problem-solve and connect to other mainstream services.
