- To place an order through the WHALES Death Registration System, please use the death record comments field to communicate order requests (must include the number of each type of certificate).
- For all other orders, email or fax a completed Certified Death Order Form – Funeral Homes

 | Payment for Certificate Orders
- For funeral homes that have an approved billing account, invoices will be sent monthly.
- All other funeral homes must provide payment at the time the order is placed.

 | Correcting and Ordering Corrected Death Certificates- An Affidavit for Correction must be emailed or mailed to the state:
- Send the original incorrect certificates, the completed CCHHS order form, and a copy of the Affidavit for Correction form to the Cowlitz County Health & Human Services office. Incomplete orders or orders without original certificates will not be accepted.
- Mail (this may delay the order): 1952 9th Avenue, Longview, WA 98632
- Bring into the office: 1952 9th Avenue, Longview, WA 98632
- Upon receipt of the completed order, CCHHS staff will monitor the record and issue the corrected certificates when available.

Notes |
- In most cases, orders will be available for pick-up the next business day after 2:00 p.m.
- CCHHS can mail certificates to the family or to the funeral home. Mailing fees may apply to each order. Orders may also be picked up in the office during regular business hours by authorized funeral home staff.
- Payments cannot be accepted by phone.
- Payment can be made in person via debit or credit card at CCHHS.
- Third-party convenience fees apply to all debit or credit card payments.
