- Online Orders: Visit www.vitalcheck.com for birth & death certificates.
- Mail-in Orders: Print and complete Vital Records Form and mail form with payment (check or money order, see current fee on our Fee Schedule) to the address provided on the order form. The order will go out next business day via regular mail.
- Walk-in Orders: Visit the Cowlitz County Health & Human Services Department. The order will be available next business day after 2:00 p.m. We are located at 1952 9th Avenue, Longview, WA 98632
