1. How can I get a copy of a birth or death certificate?

• Online Orders: Visit www.vitalcheck.com for birth & death certificates.
• Mail-in Orders: Print and complete Vital Records Form and mail form with payment ($25 each, check or money order) to the address provided on the order form. The order will go out next business day via regular mail.

• Walk-in Orders: Visit the Cowlitz County Health & Human Services Department. The order will be available next business day after 2:00 p.m. We are located at 1952 9th Avenue, Longview, WA 98632

3. Where can I get my water tested?

The Cowlitz County Health Department does not provide water testing services, however in Cowlitz County there are a couple of labs that do provide services. Contact information for the labs and drop-off sites are linked below.
