Work Release & Electronic Monitoring Requirements
Cowlitz County Offender Services must receive the listed pre-qualification information 7 to 10 days prior to sentencing or jail commitment date. Without receiving the necessary information listed below, successfully qualifying for Work Release or Electronic Monitoring is greatly reduced.

A. 
Completed Offender Services Work Release/Electronic Monitoring application DOWNLOAD HERE
B. Letter from employer/supervisor on company letterhead.
Letter is to include:
  1. Type of work you do
  2. Your work schedule
  3. Rate of pay
  4. ***Copy of the company business license and/or company financial records may also be required
C. Copies of your two most recent Pay Stubs.
D. Transportation Information
If transporting yourself:
    1. Your drivers license
    2. Proof of liability insurance on your vehicle
If some one else is transporting you:
    1. Their drivers license
    2. Proof of liability insurance on their vehicle
E. If self employed:
  1. Copy of your business license
  2. Recent financial records
  3. List of pending jobs/work
F. If you have a past history of substance abuse and/or your current offense is drug or alcohol related you will be required to get a drug and alcohol assessment and follow the recommendations of the assessment. At minimum you will be required to attend two NA/AA meetings a week and provide verification of attendance.
G. If you are on District Court Probation you are required to remain in compliance with your reporting requirements.
H. If you have past fee’s or fines due at Offender Services, Cowlitz County Jail, or Superior Court, you will be required to make a reasonable effort to make financial arrangements to pay these in full.


Pre-Screening Appointment:
Please bring all of the documents listed above, IN PERSON, plus any other required information to the Offender Services office to set up a pre-screening appointment.

IN ADDITION TO SENDING YOUR INFORMATION VIA FAX OR POSTAL SERVICE, YOU ARE REQUIRED TO CONTACT OFFENDER SERVICES TO SET UP A SCREENING APPOINTMENT. FAILURE TO SET UP A SCREENING APPOINTMENT IS CONSIDERED A FAILURE TO QUALIFY.
