Mumps Information
SUSPECT and CONFIRMED cases of Mumps are reportable to Cowlitz County Public Health within 24 hours.

The following information is intended to be resources for providers and other healthcare professionals.


DOH Mumps Outbreak Toolkit for Providers
This toolkit educates healthcare service providers about mumps prevention approaches and contains materials for patients.  It also provides steps to take during the mumps outbreak in winter 2016-2017. Intended users are healthcare providers and workers, such as hospitals, clinics, and medical staff.

Suspect Mumps Worksheet
This worksheet is for healthcare providers to report suspect mumps cases to Cowlitz County Public Health, the first step in requesting testing through Washington Department of Health.  Once completed, please fax to Cowlitz County Public Health at (360) 425-7531.

DOH Lab Virology Form
When requesting testing through Washington Department of Health, this form needs to be filled out and sent with specimen.
