Our local and state communicable disease surveillance systems rely on partners like you. The information obtained through case reports from physicians and other health care providers are an essential component to the health of the community. This type of clinician based surveillance is public health's best source of information.

In addition to the diseases listed as "Notifiable Conditions", it is vital that clinicians also report unusual clusters of disease or events that raise a level of suspicion. Your vigilance in observing and reporting is important and appreciated.
