Description
of the Program
Case management helps HIV/AIDS
patients enhance the quality of their lives through continuity of
care. A social worker from the Cowlitz County
Health Department manages each individual's particular health concerns, in
relation to his illness. The social worker may meet with client by
appointment at the Cowlitz County Health Department or visit him or
her at home to discuss the individual's particular health care and
other needs.
The social worker then coordinates
with other agencies and health care providers to obtain services,
housing, financial aid and supplies for the client.
